To schedule an appointment, please call (614) 338-8833. We accept same day appointments and afternoon walk-in appointments between 1-4pm.
New patients or established patients that have changes in address, telephone number or insurance are asked to arrive 15 minutes prior to their appointment times, in order to complete the necessary paperwork and allow for changes in our computer system.
Please bring your insurance card and co-pay to each office visit.
- We require co-pays be paid at check-in, prior to your visit.
- We accept all major commercial insurance plans and Medicare.
- Please contact your insurance company to verify we are on your particular plan.
- If you have Medicare, please bring your pharmacy insurance drug card.
Please bring ALL medication BOTTLES, inhalers, insulin, over-the-counter medications and glucose meter, if you’re diabetic.
Please have someone accompany the patient if he or she is not able to fully understand and carry out medical recommendations, or if the patient is unable to communicate clearly to our staff.
If you’re having a physical exam and/or coming in for cholesterol or glucose testing, you’ll be required to fast for 12 hours.
- Water, BLACK coffee and medications are okay while fasting
- No insulin when fasting
If you have a medical emergency, you should call 9-1-1. If you have an urgent problem, you should call our office immediately at (614) 338-8833.
